EFMP Paperwork & School Forms

 Please allow a 2-week notice for EFMP paperwork & school-form needs. Please also note there is an administration fee for the practice which covers a variety of forms and services that insurance companies don’t pay for. To cover that administration, we charge an annual fee of $40 per child or up to a maximum of $100 per family. We know this might be an unexpected charge and can be added to the patient portal to be paid at a later time.
